FEE STRUCTURE
Initial intake/assessment: $200
Individual counseling session: $180
Payment: Payment is expected at the time of your visit (unless I am billing your insurance) and can be made by cash, check or credit card. VISA, MasterCard, American Express and Discover are accepted. I offer a slightly lower fee for those paying out of pocket. Please ask Kate for details.
Insurance accepted: Aetna, Regence BlueCross BlueShield, and Pacific Source. I am an out-of-network provider for the following; Moda, Providence, UBH and Care Oregon/Healthshare.
Cancellation Policy: I allow one free "no show." If you have a true medical or family emergency, I will not charge you for a missed appointment. In other cases, if you do not show up for your scheduled appointment and have not notified me at least 24 hours in advance, you will be charged $100.00, even if I am billing your insurance since insurance companies do not reimburse for missed appointments.
